D.  Contents of Consent Document


Guidelines for State Courts: Indian Child Custody Proceedings, 44 Fed. Reg. 67584, 67593 E.2
The consent document should include:
  • The name and birth date of the Indian child, the name of the Indian child’s tribe, any identifying number or other indication of the child’s membership in the tribe, and the name and address of the consenting parent or Indian custodian;
  • Consent for a foster care placement should also include the name and address of the person or entity by whom the placement was arranged, if any, or the name and address of the prospective foster parents, if known at the time;

Consent to termination should also include the name and address of the person or entity or through whom any preadoptive or adoptive arrangement has been or is to be arranged.
